This Consent to Telehealth Treatment explains how Brand Merchants Network LLC ("Yucca Health," "we," "us," or "our") provides healthcare services through telehealth and what it means for you as a patient. By agreeing to this document, you acknowledge that you understand and consent to receive medical services via telehealth.
Nature of Telehealth Services
- Telehealth involves the delivery of healthcare services using electronic communications, including secure messaging, audio, and video when applicable.
- Licensed healthcare providers will review your medical history, symptoms, and treatment requests to determine whether a prescription or therapy is appropriate.
- Providers may review your intake and communicate with you asynchronously (not in real time) or synchronously (live chat or video), depending on your case and provider availability.
- Yucca Health facilitates access to providers but does not itself practice medicine. All medical decisions are made by licensed providers.
Eligibility
- You may qualify for treatment if your Body Mass Index (BMI) is 30 or higher, or 27 or higher with at least one weight-related condition (such as high blood pressure, type 2 diabetes, high cholesterol, obstructive sleep apnea, or cardiovascular disease).
- If your BMI is above 25, you may still be at risk for health issues and could qualify depending on your overall medical history. Eligibility is determined by a licensed healthcare provider after reviewing your intake.
Risks and Limitations of Telehealth
- Telehealth is not a substitute for in-person medical care in all situations.
- There may be limitations in diagnosing or treating certain conditions without an in-person exam.
- Technical failures may disrupt services.
- Emergencies cannot be managed through telehealth. If you experience a medical emergency, you must call 911 immediately.
Compounded Medications Disclosure
- Some prescriptions may involve compounded medications, which are not approved by the U.S. Food and Drug Administration (FDA).
- Compounded medications do not undergo the same safety or efficacy testing as FDA-approved products.
- A licensed provider will determine if a compounded medication is appropriate for you.
Patient Responsibilities
By consenting to telehealth services, you agree to:
- Provide accurate and complete medical information.
- Inform your provider of any changes to your health or medications.
- Follow the treatment plan and instructions given by your provider.
- Use secure and private internet connections for telehealth services.
Privacy and Confidentiality
- Your information will be collected, stored, and shared in compliance with the Health Insurance Portability and Accountability Act (HIPAA).
